Patients want access, and they want it in a variety of ways.

At Hoag, we are finding exciting ways to bring high-quality care directly to the communities we serve. We are in the throes of a multifaceted plan that includes physical growth and an emphasis on innovation to better meet the needs of our patients and connect them with their physicians as quickly and conveniently as possible.

This summer, we will open Hoag Health Center Irvine-Sand Canyon to provide a wide array of specialized, high-quality healthcare services to the Irvine community. [Those include] outpatient surgery, specialty care, women’s services, a full-service imaging center, cancer center, lab and pharmacy to complement Hoag Irvine’s acute care.

Hoag is also opening more urgent [care centers] throughout Orange County to improve access. We recently celebrated the opening of our urgent care at Hoag Health Center-Aliso Viejo and look forward to the opening of locations in the next few months at Los Olivos in Irvine and Hoag Health Center Irvine-Sand Canyon.

Those facilities break the mold of what patients typically might expect from an urgent care, offering expanded specialized services along with access to primary-care physicians. They serve as an all-encompassing wellness center that handles everything from health sceenings, annual exams and vaccinations to treatment of colds, flu symptoms, fractures and nonemergency services.

We are taking this concept of delivering high-quality services in the most expedient way possible another step further with tele-health. Hoag is creating a network of specialists available to interface with patients for consultations using secure video conferencing, allowing patients to interact quickly with the best medical professionals and specialists, regardless of their location.

Our leaders and clinicians are continually exploring new technologies to provide services such as remote patient monitoring, medication modifications and health education.

The convergence of those ideas provides Hoag the opportunity to deliver care in a very efficient way. It is a new way of thinking and a new approach to healthcare that takes resources, planning and expertise to execute, all of which Hoag is dedicated to, [to] remain on the forefront of patient care.

At Kaiser Permanente, we believe in equal access to healthcare for everyone. In 2015, we took a significant step in that direction as more people obtained care and coverage through the broadening implementation of the Affordable Care Act and California’s healthcare exchange. Last year, our membership in Orange County grew to more than 520,000, reflecting annual growth of 7.9%. This year, we anticipate continued positive membership growth and have added physicians and nurses to ensure access to needed services. As we continue to deliver on our mission to provide high-quality, affordable care, we are honored and privileged that so many Orange County residents have entrusted Kaiser Permanente to support their total health.

Looking to the future, we have created an innovative way to provide enhanced outpatient care to produce more effective health outcomes. That lies within a vision that shifts the center of care from Kaiser Permanente medical offices directly into our members’ daily activities, relationships and communities. We call it “Life Integration,” where we weave healthcare in a more meaningful way into people’s lives. Instead of asking members to work around our system, we will bring healthcare to members in new and varied means.

To advance the “Life Integration” approach, we have developed a healthcare experience that looks and feels completely different from any medical visit members have ever had. We call it “Re-Imagining Ambulatory Design” or Project RAD. New features and technology will include digital membership cards and express check-in, mobile physician work-stations, tele-consultation capabilities, and reception areas that educate, entertain and reinforce Kaiser Permanente’s Thrive brand. The first Project RAD facility in Orange County will be in the Fullerton La Habra area in 2017.

As a leader in innovation and health transformation, Kaiser Permanente has made significant progress in virtual care delivery and tele-health initiatives, such as telephone appointments, KP NOW 24/7 virtual visits, integrated video visits, and programs in tele-stroke and tele-derm. Members can now log into their accounts with their thumbprints on their iPhones, and use the KP mobile app to make appointments, see test results, check immunizations, and enter relevant personal data before the visit.

In addition, we continue to be partners in good health with thousands of businesses in Orange County, providing personalized care, free online programs and wellness coaching supporting healthier tomorrows for their employees.

Those achievements reflect the tremendous work and dedication of our more than 7,000 employees and 1,000 physicians who come to work every day to deliver better health for all.

Children’s Hospital of Orange County

This will be an exciting year for CHOC Children’s and for the children and families we serve. Dedicated to building and supporting the healthiest community for children, we are advancing a coordinated pediatric system of care focused on proactive illness prevention, in addition to chronic, acute and post-acute care.

A $17.7 million grant from the Centers for Medicare & Medicaid Services to CHOC in partnership with Rady Children’s Hospital [in San Diego] will help Southern California pediatricians expand their quality improvement capacity, learn from one another, and achieve common goals of improved care, better health and reduced cost.

Recognizing the unmistakable link between mental and physical health, we are making tremendous progress on our vision for a mental health system of care. In addition to expanding outpatient mental health services and hiring additional specialty staff, including pediatric psychiatrists, construction will be well under way this year on the CHOC Children’s Mental Health Inpatient Center. A $5 million donation from Sandy Segerstrom Daniels serves as the cornerstone of a broader fundraising campaign to raise $11 million for inpatient capital and startup costs, and $16 million to endow the program.

Construction will also begin this year on the expansion of CHOC’s neonatal intensive care unit to bring 36 private patient rooms with family rooming-in facilities to our main campus in Orange. While our NICU is among the best in the nation, we know we can do more. Private rooms are associated with improved clinical outcomes and align with our commitment to patient- and family-centered care.

The patient experience for adolescents and young adults with cancer is one of the driving forces behind our new wing for those patients. Expected to open this spring, the space will feature a patient lounge and four private rooms, each with an Infusionarium intended to provide an immersive healing environment for this unique patient population.

Our genomic sequencing and precision medicine efforts, thanks to the generous support of Hyundai Motor America, will be enhanced through partnerships with Caris Life Sciences and the California Kids Cancer Comparison Initiative. Exciting research in 2016 includes a clinical trial investigating the treatment of persistent acute lymphoblastic leukemia.

CHOC’s laser focus on innovation will only increase this year through the Sharon Disney Lund Medical Intelligence and Innovation Institute, and the Innovation Institute. Our goal is to transform pediatric medicine, securing the healthiest and brightest futures for children.

In 2016, Orange Coast Memorial Medical Center is continuing to reinvent our campus in Fountain Valley to more actively engage our patients and their families in their own health and well-being. From new and expanded services and facilities to preventive care, innovations and advanced technology, our goal is to greatly impact the wellness-focused lifestyles of our community.

Every program we offer is built around enhancing community wellness, supporting healthy initiatives and improving patient outcomes.

Our transition from inpatient care to comprehensive hospital, outpatient and community care became a focus when we opened the Orange Coast Memorial Patient Care Pavilion in 2009.

This modern facility now includes a comprehensive cancer center, full-capability imaging center, lab services, advanced radiation oncology treatment, Orange County’s only hospital-sited CyberKnife radiosurgery delivering pinpoint-accurate radiation in a shorter treatment cycle, and specialty physicians’ offices. 

This fall, we [will] open the Orange Coast Memorial Health and Wellness Pavilion, a 46,000-square-foot renovation and redesign on our campus that will house the Center for Spine Health, an outpatient pharmacy, Wellness Resource Center, conference center for community activities and education, as well as physician offices.

We will unveil the Center for Spine Health [this year]. With neck and back pain affecting 50 million Americans, our team of expert pain management specialists, specially trained nurses, therapists and surgeons will provide comprehensive, coordinated care designed to help reduce and eliminate pain and get patients back to life and doing what they love.

Our new Digestive Care Center provides groundbreaking care for patients suffering with gastroesophageal reflux disease, motility disorders and benign and cancerous diseases of the upper gastrointestinal system. Orange Coast’s new special technology for gastroesophageal reflux treatment helps restore the body’s natural barrier to reflux, along with comprehensive diagnostic and treatment services.

In keeping with our commitment to provide highly individualized care with rapid recoveries, Orange Coast remains at the forefront of minimally invasive surgery expertise by offering an increasing array of treatment choices aimed at providing personalized care with less pain, minimal scarring and a quicker return to active living. This care is offered for gynecological, urological, lung and gastrointestinal disorders, as well as cardiac treatment.

As a member of MemorialCare Heart and Vascular Institute, we perform more cardiac robotic-assisted surgeries than any other healthcare system in the region and will continue to expand our efforts in this area this year.

MemorialCare Medical Foundation

MemorialCare Medical Foundation is unveiling several new healthcare centers, innovative programs, progressive partnerships and comprehensive services [in an effort to continue] substantial growth and readiness to meet the needs of patients and families.

We are comprised of MemorialCare Medical Group and Greater Newport Physicians Independent Practice Association and are championing critical initiatives that deliver the integrated, preventive and outpatient care that keeps people healthy, along with the right diagnoses and treatments to care for our communities.

MemorialCare Medical Foundation continues to grow, providing 350,000 patients with a choice of more than 350 primary care physicians and 2,200 specialists in dozens of locations throughout the region.

We recently opened a multilevel health center in Fountain Valley as a joint venture with more than 50 GNP physicians featuring family care centers, urgent care, laboratory, physical therapy, imaging, diabetes programs, anticoagulation clinic and specialty services. We built a 25,000-square-foot, “one-stop” primary care health center with room for several new physicians in San Juan Capistrano.

We are adding practice locations in Rancho Mission Viejo, Long Beach, and another 25,000-square-foot healthcare campus in Huntington Beach with several primary care physicians, advanced imaging, laboratory services and urgent care, as well as a large, comprehensive outpatient facility on Orange Coast Memorial’s campus.

Our eight MemorialCare Surgical Centers, where 496 surgeons have performed outpatient procedures, and 10 free-standing MemorialCare Imaging Centers throughout the region are increasing in numbers and capabilities. We are expanding the important collaboration with our independent physician partners, Monarch Healthcare, and other medical groups and healthcare providers.

Recently, a MemorialCare Regional ACO subsidiary was chosen among 21 accountable care organizations nationally to participate in the Centers for Medicare & Medicaid Services Next Generation ACO model based on our significant experience in high-quality, coordinated care. Similar arrangements with the Aetna and Anthem health plans resulted in 250,000 patients and families affiliated with our medical groups accessing added benefits to improve their health and wellness and ensure comprehensive continuity of care. These include care navigators that coordinate all aspects of care in inpatient, outpatient, physician offices and community-based locations; assistance during transition from an inpatient hospital setting to the patient’s home; and home visit teams for frail patients.

For the ninth year, Greater Newport Physicians and MemorialCare Medical Group were honored by California Association of Physician Groups with the Standard of Excellence program’s highest ratings. In 2016 and beyond, we will continue to expand our reach with additional choices for patients—more primary care offices and convenient, cost-effective ambulatory care options.
