Landmark Health Expands Home-Based Care

Landmark Health LLC, a home-based medical care provider in Huntington Beach in its fourth year, has expanded its care delivery platform with the newly rolled out Landmark Connect. Services are now available to people living in skilled nursing, independent living and assisted living facilities, as well as continuing care retirement communities and other long-term care facilities.

The program is available only in New York, but will launch in California at the end of the year, said Callae Snively, executive director of operations.

“We provide care for more advanced acute patients in their home settings. It is really important to us to support these patients in their home settings—‘home’ might not necessarily be in a home setting, those needing post-acute and long-term care may be in assisted living and skilled nursing facilities—that was why we chose to expand into other facilities,” Snively said.

Landmark Health’s physician-led medical teams provide house calls and care to individuals with multiple chronic conditions, including medical, behavioral, social and palliative care.

It has two separate business models. Its parent model provides care to eligible patients who have long-term care needs. Eligibility is determined by insurance plans that the company contracts with, and costs are covered by those plans. The new model doesn’t require patients to be deemed eligible by insurers.

Landmark Health has about 45,000 patients in its parent model and another 1,000 in its new program, according to Snively.

Jennifer Gilchrist has been named president of the new program. She joined Landmark Health in January 2015 as vice president and general manager of the market in Albany, New York, and most recently served as regional president of East Coast operations.

Education

Irvine-based Freedom Innovations LLC introduced the Freedom Institute of Technology, an educational program for clinicians comprised of live trainings, webinars and online courses. The complimentary resources include information on products, new technology and reimbursement support.

The company makes lower limb prostheses and is listed on the Business Journal’s list of medical device companies.

“We feel it’s important to offer a robust educational program to all of our clinician partners … either in person … or through online interactive learning,” said Chairman and Chief Executive David Smith. Smith joined Freedom Innovations in April 2016. He previously served as a partner of healthcare-focused private equity firm Health Evolution Partners in San Francisco.

Freedom Innovations has over 30 products in its portfolio, including carbon fiber and fiberglass feet, microprocessor ankles, microprocessor knees and hydraulic ankles. Its microprocessor ankle Kinnex, which is manufactured in Irvine, will launch soon in Europe. Freedom Innovations products are equipped with Wi-Fi to support clinical reporting.

The company has 158 employees—including its European headquarters in the Netherlands—with 51 based at its global headquarters in Irvine. It makes all of its prosthetics in the U.S. and has manufacturing sites in Irvine and Gunnison, Utah.

Packaging Buy

ProPlas Technologies in Garden Grove, which makes injection molding and mold making for consumer goods and medical devices markets, was acquired by packaging firm Comar LLC in Voorhees, N.J. for an undisclosed price. Comar is a specialty packaging and dispensing provider of bottling services for prescriptions, topical products, specialty foods, household and industrial cleaning supplies.

The acquisition is intended to strengthen Comar’s ability to service the diagnostics and medical device market.

“Comar’s recent investments in a class 8 cleanroom and new innovation design center, combined with the acquisition of ProPlas, will position the business to execute on its growing strategy,” said Adam Piatkowski, managing principal of Graham Partners in Los Angeles. The private investment firm owns Comar.

ProPlas has a 42,000-square-foot facility that includes two soft-walled cleanrooms, 26 molding machines and five high-speed CNC machines. In addition to production molding and assembly, the company also offers design for manufacture expertise, and in-house mold making.

Huntington Beach-based CiceroDx said the company has launched its first product, a test panel for unexplained infertility to be marketed under the name ReceptivaDx. The company is in the field of women’s health diagnostics, particularly unexplained infertility and failed implantation. … PrescribeWellness introduced PharmacyGrowth, a platform that aims to improve patient engagement. The software developer builds platforms to better connect patients with community pharmacies. … Supplement maker ChromaDex Corp. has signed an exclusive worldwide patent license and research agreement with the Scripps Research Institute in La Jolla, one of the world’s largest private nonprofit research organizations, to study its anti-aging ingredient, Niagen, or nicotinamide riboside.
